About PATH

PATH is the leader in global health innovation. An international nonprofit organization, we save lives and improve health, especially among women and children. We accelerate innovation across five platforms—vaccines, drugs, diagnostics, devices, and system and service innovations—that harness our entrepreneurial insight, scientific and public health expertise, and passion for health equity. By mobilizing partners around the world, we take innovation to scale, working alongside countries primarily in Africa and Asia to tackle their greatest health needs. Together, we deliver measurable results that disrupt the cycle of poor health. Learn more at www.path.org.

PATH’s work in Vietnam began in 1980 with a project to improve local condom manufacturing capacity. Other projects in reproductive health followed, and in 1997, we opened a permanent office in Hanoi. We broadened our focus from contraceptive technologies to wider health issues, including emerging and epidemic diseases; vaccines and immunization; reproductive, maternal, and child health; and digital health solutions. For more than 35 years, we have collaborated with a diverse range of partners to help improve health in Vietnam.

The Communities for Healthy Hearts Project

PATH is collaborating with the Novartis Foundation, a philanthropic organization pioneering innovative health care models, to introduce an innovative health care delivery model designed to improve hypertension management and control in Vietnam.

Implemented by PATH in collaboration with the Ho Chi Minh City Provincial Health Department and Ho Chi Minh City Preventive Medicine Center, the three-year program aims to improve blood pressure control among adults by increasing access to and use of high-quality hypertension services that are sustainable and scalable. The objectives of Communities for Healthy Hearts are to:

  1. Increase awareness of hypertension and demand for screening and treatment among adults. ?

  2. Increase availability of hypertension screening and referral services. ?

  3. Increase availability of hypertension services, reflecting the full continuum of patient-centered care in public and private facilities. ?

  4. Reduce loss to follow-up by strengthening referral systems, increasing patient contact, and empowering patients. ?

  5. Enable country-level decision-makers to use project evidence to sustain and scale hypertension services. ?

The Communities for Healthy Hearts model is implemented in four districts of Ho Chi Minh City: Thu Duc, Go Vap, District 8 and District 12. These four districts cover a population of approximately two million people, and a target population of approximately 700,000 people aged 40 years or older.
