The Fred Hollows Foundation  is seeking expressions of interest from individuals, consulting companies, or research institutes for project baseline and endline survey and training on occupational eye health care and action plan developing for eye health care in factories as attached TOR.

The applications should include:

• A project proposal for baseline and endline survey (using the template provided in Appendix 1).
• A proposed occupational eye health care training agenda for group workers’ leaders and consultancy rate and a list of key interventions for an action plan of occupational eye diseases mitigation.
• A cover letter addressing the key selection criteria and including two references.
• The most updated curriculum vitae for individual consultants or the Principal Investigators if applying as a team.
• If applying as a team, please also provide a team capacity statement which describes the team, collective experience and track record of working collaboratively.
• Two examples of previous written work (e.g. published reports)
