For consultant to support the eHTN.Tracker implementation

1. Background

The Communities for Healthy Hearts (CH2) Program is an innovative healthcare delivery model designed to improve hypertension management and control, which focuses on Ho Chi Minh City (HCMC), home to the largest urban population in Vietnam. CH2 is implemented in four districts, which cover a population of approximately 2 million people.

CH2 aims to improve blood pressure control and health outcomes of adults living in urban settings in Vietnam by:

  • Increasing awareness of hypertension and demand for screening and treatment among adults.
  • Increasing availability of hypertension screening and referral services.
  • Increasing availability of hypertension services reflecting the full continuum of patient-centered care in public and private facilities.
  • Reducing loss to follow-up by strengthening referral systems, increasing patient contact, and empowering patients.
  • Enabling country-level decision-makers to use project evidence to sustain and scale hypertension services.

2. Scope of work:

The consultant will support the project to implement and strengthen application of an electronic tracker for hypertension management (eHTN.Tracker) at primary health care levels (commune health stations, district health centers, and private poly-clinics) across the four CH2 project districts. Specific duties and responsibilities include:

  • Work with CH2’s mHealth team to develop a plan to roll-out the eHTN.Tracker among approximately 70 health care providers.
  • Provide technical support, field visit monitoring, coaching, mentoring, and on-the- job training to ensure health care providers under the project network fully implement and apply the system.
  • Provide weekly updates for the monitoring and evaluation officer on the progress and challenges of implementing the eHTN.Tracker, and work to resolve any issues.
  • Consult regularly with the mHealth software developer and CH2 team at PATH to assess, adjust, adapt, and finalize the utilities and functions of the eTHN.Tracker in collaboration with and based on the feedback of the field implementation.
  • Co-facilitate the mHealth and eTHN.Tracker training course for health care providers.
  • Demonstrate best practices and lessons learned regarding the eHTN.Tracker implementation in a dissemination workshop.

4. Location of work: Ho Chi Minh City

5. Requirements

  • A bachelor degree or higher on public health, M&E, social science or related fields
  • Have a software and application development certificate or higher
  • Experience in developing and coding software, especially web-based database applications
  • Good communication/presentation and/or training skills
  • Experience in health care, public health, and/or social development project implementation and execution is a plus
  • Understanding of health care/hospital information systems is a plus
