Research/Consultant Agency to Collect Data and Make Field Work Report Following PSI Objective in 8 PEPFAR Provinces

PSI Vietnam is now seeking a qualified research/consultant agency to collect data and make field work report following PSI objective in 8 PEPFAR provinces (Hanoi, Nghe An, Quang Ninh, Lao Cai, Dien Bien, Hai phong, An Giang, Can Tho & Ho Chi Minh city) . Could you please study the details in the attached files and send us your technical proposal & quotation paying attention to the followings:

1. Use PSI quotation FORMAT (PSI – Service – 2012 – 085 attached), including file “financial proposal _ IDU survey” for your quotation

2. Properly sign and stamp your quotation & technical proposal

3. Indicate outside the envelop with: Proposal “Conduct behavior survey among male IDU in 8 Pepfar provinces – No. 2012-085” & Quotation “Conduct behavior survey among male IDU in 8 Pepfar provinces – No. 2012-085”

4. Send your technical proposal & quotation separately via one of the followings:

- Hand-deliver it to PSI office at 4th Floor, Side B, Toserco Building, 273 Kim Ma, Hanoi.

- Send it via post to the above-mentioned address

The deadline for submitting technical proposal & quotation is 7th September, 2012. Proposal & quotation which are not submitted by this deadline will not be considered.
