PSI Vietnam is now seeking a qualified research agency company to conduct the Malaria Mystery Client Survey and Focus Group Discussion in 4 provinces (Dak Lak, Gia Lai, Binh Phuoc and Khanh Hoa). Please refer to attached files for the details and pay attention to the following:

1. Properly signed and stamped your quotation & technical proposal

2. Submit Financial proposal (quotation) and Technical proposal in 2 separated and sealed envelopes with title “SVC20-13 Malaria Mystery Client Survey and Focus Group Discussion in 4 provinces ”

3. Send your technical proposal & quotation separately via one of the followings:

- Hand-deliver it to PSI office at 11st Floor, VinaFor Building, 127 Lo Duc, Hai Ba Trung, Hanoi.

- Send it via post to the above-mentioned address

Note: We will evaluate your technical proposal based on following criteria:

     *   Quality of proposed plan and methodology
     *   Organization profile and Proposed personnel and management approach
     *   Timeline
     *   Working Partnership

The deadline for submitting question, clarification is 12:00 September 28, 2020 . All questions must be sent to email address procurement at psi.org.vn<mailto:procurement at psi.org.vn> and honghoa at psi.org.vn<mailto:honghoa at psi.org.vn> . PSI’s clarification will be shared to all interested agencies

The deadline for submitting technical proposal & financial proposal is 16:00 October 5, 2020. Proposals which are not submitted by this deadline will not be considered.

  *   Scan of business registration
  *   Copy of similar contract for event organization
